I think C & D, because Microsoft doesn’t offer any free upgrade since 31 December 2017 through Windows Update anymore.

C & E are correct but E is outdated because the free upgrade offer expired. D is wrong, you can’t perform an upgrade installation from a booted USB device, it will tell you to boot Windows 8.1 and start the upgrade installation from the setup.exe. Only custom installations work when booted from USB device.
