Yes, at first sight the problem looks like a incorrect zone configurarion… however, the question said “the switches detect BOTH HBA ports”. Asuming that “both” switches can detect “both” HBA ports at teh same time, you will need a “inter switch link” (ISL) between the switches to achieve it, so maybe that’s the reason why we can “asume” that answer D is correct…
“the switches detect BOTH HBA ports” phrase does only says that connectivity between server and switch is OK. It does say nothing about zoning between each HBA and storage array. So I think that answer D is really not MOST likely cause.
