It doesn’t make sense to me, D being the correct answer.
It asks:
“You are about to apply a patch to a database. Which recommended action is incorrect?” – So it means you haven’t applied any patches to the database YET, and to point out THE incorrect action.

I think it should be E – “E.
You should stop all applications running from the software directory that is being patched.”

the question asks which recommendet action is INCORRECT. This is a typical problem with questions in Oracle tests. Double or triple negations.

the action “check if the patch is applied” is incorrect. So point “D” is the correct answer.
