A is wrong as it CANT be done at the same time
B is possible in 5. Option: “Enable parallel remediation for hosts in the selected cluster”
C is wrong. Tools upgrade require reboot of the VM
D Host UPGRADE needs reboot

so the correct one is B

I think the question in general is incorrectly worded. I believe there are actually two answers according to the vpshere-esxi-vcenter-server-50-upgrade-guide.pdf on page 145.

“An orchestrated upgrade of virtual machines allows you to upgrade VMware Tools and the virtual hardware for the virtual machines in your vSphere inventory at the same time.”

And for ESXi hosts on page 96:
“You can use Update Manager to perform orchestrated upgrades of the ESX/ESXi hosts in your vSphere inventory by using a single upgrade baseline, or by using a baseline group.”

Thus I can’t see how A cannot also be a valid answer. So I think it should be B AND A.

A is the answer since the host should be set to maintenance mode which can not be realistic if most hosts are upgraded simultaneously

Yes, I will also opt for A as per upgrade guide
