At first I didn’t get why there needs to be a “green to yellow” action because there is “yellow to red” action. But then I realized that the alarm might go from green to red instantly and in that situation there is no “yellow to red” phase at all. Am I correct with my assumption?

Negative. Alarms Must go from Green to Yellow and Yellow to Red.. They can never go from Green to Red or Red to Green. Since the status is Green by default and the alarms only account for Yellow to Red conditions – it would never trigger.
