how is possible the virtual machine be restarted by the cluster after missing three times heartbeating and no error/alarm message are displayed at vcenter?

Surely the correct answer is A and C. If the HA cluster was configured with all the defaults then VM monitoring would not be enabled.

By default VM monitoring is disabled, only hosts are monitored.
