I think the third item should be yes, since no alternate database server is configured. A SQL Server failover cluster is referred to by a single name. If you don’t want to change the service applicaton configuration, you must create a SQL Server Failover Cluster.

PerformancePoint Services database supports both synchronous mirroring with SQL 2008 R2 & 2012, and asynchronous mirroring or log-shipping with SQL 2008 R2
